Procura Launches Clinical Pathways to Help Improve Outcomes in Aged and Community Care
Brisbane, Queensland – Procura announced the launch of their Clinical Pathways software in Australia today. Based on the VNA FIRST Home Care Steps® protocols, the pathways provide a standard care plan that ensures consistency in care. These evidence-based clinical pathways have proven to be successful in decreasing hospital readmission, reducing disease exacerbation, and improving client education.
“The Clinical Pathways software allows our customers to standardise their clinician’s documentation and ensure that it’s evidence-based,” says Warren Brown, President of Procura. “Based on an industry standard library of outcome driven pathways, they encourage consistency in outcomes through appropriate goals and interventions from visit to visit.” He continues, “As each organisation also has the ability to design pathways that are unique to them, they are able to bring their own best practices forward in a uniform manner.”
Clinical Pathways allow clinicians to identify at a glance what a client has achieved in the pathway, what the next steps are and what problems are still present. Interdisciplinary case management and outcomes tracking assist in reducing patient risk and promotes consistent, high quality care delivery.
The Step by Step client education guides that accompany the pathways empower clients to prepare for visits and provide motivation for sustained compliance with disease management lifestyle changes.
“We recognize the need for Aged and Community Care organisations to maximise the efficiency with which they deliver effective care,” comments John Lacy, National Account Executive, Procura. “This tool is designed to help nursing and allied professional tailor the care provided to their clients’ primary and co-morbidity health conditions and demonstrate that they are pro-active partners in disease management.”
The VNA FIRST Home Care Steps® pathways have been licensed to over 1000 home health and hospice organisations and incorporate 87 pathways that include both primary and secondary diagnoses and cover multiple disciplines. Procura extends the benefits of the pathways with a Pathways Designer that allows customers to include additional clinical elements, or design additional pathways to address organisation specific criteria.
